Secure Online Payment

1. Choose Florida for Florida clinic patients or Kentucky for Kentucky clinic patients.
2. Input the amount of your payment and the security code shown on the page, then select Continue.
3. Input your Account # from your statement in the Invoice Number field.
4. Input the Patient Name in the Description field.
5. Input the Card #, Expiration Date and CVV.
6. Input the name, address, ZIP and phone number as shown on the billing statement.
7. Input the email address for the payment receipt, then click Pay Now.
