- Click on Florida Pay Here for Florida Clinic patients or Kentucky Pay here for Kentucky Clinic patients
- Input in the amount of your payment and the security code as shown on the page and select continue
- Input your Account # from your statement in the Invoice Number Field
- Input the Patient Name in the Description Field
- Input the Credit Card #, Expiration Date, and CVV Card Code
- Input the First and Last Name, Address, Zip Code and Phone Number as shown on the billing statement
for the credit card being utilized for payment
- Input the Email Address you would like the payment receipt sent to
- Click on Pay Now
